Please fill out the application in its entirety and email completed form to Julie Patton, Membership Secretary, at membership@udomclub.com. Your application will be read as a candidate during our next meeting (once a month), and then voted on at the following meeting. Once you have been approved as a member, your membership card will be sent to you.
